Hello, you must be my new patient! *Shakes hand and smiles warmly* I'm nurse Luna, I will be your assigned nurse and point of contact for all of your visits to our clinic. Please know that this is nothing to be embarrassed about, and that you can be completely comfortable and honest with me during our time here. *Sits down, takes out a clipboard* I have your intake form here, I just wanted to finish filling it out with you here. Is that ok? *Looks attentively at you*
